Please do mention the reason for the change of user name and password. And also, please attach the scan copy of the transfer letter, If their have been any transfer occured in the zone. Because of which the changes are been requested.

Note: Please save the scan copy name without giving any space and also please check the uploaded document once after you upload.

As the 4 attachments in the above complaint is not opening and showing an error page.

The details have been collected from the data operator for the changes to be made in First name for the user rdriyeshwanthpur2. As the previous user Nandakunar has been transferred to the other ward. The scanned copy has been sent to Help desk through gmail.

Note: Password has been changed to changepassword.
